General Fund - Payment Request Form

Please use this form for the following payments: discretionary fund expenditures, approved monthly grants, and board-sponsored grants. BHSDG votes on the use of these funds and some expenditures may require approval from an administrator or program lead.

Designated Funds - Payment Request Form

Please use this form for the following payments: small school, department, or program expenditures. The requests will be sent to the fund manager of the small school, department, or program for approval. BHSDG is the fiscal sponsor of these funds but does not determine their use.

The Berkeley High School Development Group processes reimbursements and payments from the general fund to pay for board-sponsored grants, discretionary grants, and approved monthly grants. In addition, BHSDG acts a fiscal sponsor for various programs at Berkeley High School, Berkeley Technology Academy, and Berkeley Independent Studies (9-12). These are referred to as designated funds. There are two payment request forms: one for the general fund and one for designated funds . Please review the options below and submit your payment request on the correct form. Please note the information on the top of each form which explains the process, deadlines, timing, and required documentation.

Classroom Grants for teachers have a different process and form that was distributed to the teaching staff from the administration

BHSDG does not process payments between June 11 and July 31
